Medical questionnaire: Checking suitability for preliminary tests in Israel and documentation of legal consent (according to section 5 of the questionnaire).

The contact form is stored in the Airtable system (ISO 27001, SOC 2, GDPR compliant) and in a secure email.
The medical questionnaire is saved as a PDF on the Fillout platform (SOC 2 Type II compliant, with 256-bit SSL/TLS and AES encryption).
4. Information retention time
Contact form: Information is retained for up to 3 years or until a deletion request is made, whichever comes first.
Medical questionnaire: The information is retained for at least 7 years, as required by the Patient Rights Law, and may be retained for up to 10 years for legal protection.
Cookies: are saved according to the banner settings or until you disable them.
5. Your rights
According to the law, you have the right:
Access your personal information.
Correct incorrect information.
Request deletion of information (except medical information which must be retained for 7 years).
